HELPFUL TIPS TO KNOW

Dear Parent or Guardian,

Welcome to our office and thank you for choosing us as your child's dentist. We will continually strive to provide your child the most comprehensive and optimum care. To help our doctor and team accomplish this, we would like you to read the following:

• Please have your child use the restroom before checking in for the appointment.
• Please arrive 15 minutes prior to your child's appointment to help provide relaxation or playtime. This will also allow your child to become better acclimated with the environment as well as provide the parent or guardian ample time to update your child's current medical and health information.
• If you arrive more than 15 minutes late for your reserved appointment time, the doctor will exercise discretion in determining if your child's appointment can be honored.
• Due to OSHA regulations food and drink are prohibited in our reception and treatment areas.
• A new patient form, appointment and financial policy, HIPAA information and map are available on the website. Please complete both pages of the new patient form, sign and date the Appointment/Financial policy and the HIPAA Acknowledgement, and return to our office at your upcoming appointment.
